This study was conducted in a hospital at Kota Bharu, Kelantan. The main objective of this study was to evaluate the level of safe hospital based on the structural, non-structural and functional indicators. Methods used to conduct this study were semistructured interview, walkthrough observation, document review and a set of hospital checklist. Six of the potential emergency and disaster hazards in a hospital were identified and also three indicators of safe hospital during emergency and disaster were determined. The checklist adapted by Hospital Safety Index – Evaluation Forms for Safe Hospital (PAHO, 2008) was filled out and then evaluate the level of safe hospital based on the indicators. It has been accomplished by utilizing the Hospital Safety Index (HSI) calculation. Given that each item has three levels of safety (low, medium, high) and a constant value was applied to each level of safety. The findings revealed that the final Hospital Safety Index (HSI) score places this hospital of 0.6 in category B among three existing classifications of safety. Result showed that the hospital’s current safety levels were ability of hospital to function during and after emergency and disaster were potentially at risk. In conclusion, intervention measures are needed in the short term. The efforts being made by the hospital’s disaster committee towards emergency and disaster preparedness are commendable though more still needs to be done. Healthcare institution should always be available and prepared for disaster in order to save as many lives as possible. It is recommended to continuously assess its preparedness for emergencies and disasters for identifying problems and assigning duties to various individuals in an effort to make sure that the hospital is prepared and will be available during disasters and emergencies.
