520 3 _aHealthcare facilities play a vital role in serving communities. However, they are among the most resource-intensive building types, resulting in major environmental and social impacts. Although Environmental, Social, and Governance (ESG) principles are increasingly promoted to enhance sustainability performance, Malaysian healthcare facilities lack of a comprehensive and structured ESG sustainability framework tailored to their specific operational context. Hence, this study aims to develop and validate a sustainability framework for evaluating ESG impacts in Malaysian healthcare facilities. A mixed-method approach was employed comprising Systematic Literature Review, Pilot Study, and Pairwise Questionnaire Survey. The data obtained were analysed using Fuzzy Analytic Hierarchy Process (Fuzzy AHP), and Thematic Analysis. From the systematic literature review and pilot study, six (6) sustainability factors, and twenty-four (24) sub-factors were identified. By employing Fuzzy AHP analysis, the study findings reveal that Environmental Sustainability is the most critical factor. Meanwhile, the sub-factors with major impacts including Green Building Rating System (13.06%), Enhancing and Maintaining Indoor Environmental Quality (IEQ) (8.56%), Using Sustainable Construction Material during Construction Phase (8.05%), Adopting Advanced Smart Energy Saving Solution (7.55%), Promoting Innovative Circular Economy Model (7.17%), and Utilising New Technologies for Smart Building (6.57%). Following these findings, the framework was developed and validated by five (5) experts from various professional backgrounds. The framework was evaluated based on its relevancy, practicality, and completeness. All five (5) experts agreed that the framework is practical and suitable for ESG sustainability assessment in healthcare facilities. The development of ESG sustainability framework specifically tailored for Malaysian healthcare facilities to guide researchers, healthcare practitioners, and policymakers in improving sustainability practices is the main contribution of this study. This study also contributes to sustainable development by supporting SDG 3 (Good Health and Well-being), SDG 9 (Industry, Innovation and Infrastructure) and SDG 13 (Climate Action).
